The Long Term Home Health Care Program is a unique program in New York State. Funded by Medicaid, it is a cost-containment alternative that allows appropriate people to receive nursing-home level care in their own home.

Short-term Care and Rehabilitation
We also offer a broad range of short-term in-home nursing and rehab services, funded through Medicaid and private insurance providers.

Staff
We maintain a full staff of highly skilled providers and occasional use of contracted community-based care-givers in a variety of health-care disciplines.

Telehealth
We also offer a system of electronic information-sharing and communication technology that delivers medical data through a phone line. Self-monitoring devices, such as a blood pressure unit, scale or pulse oximeter are placed in a patient’s home and hooked up to a phone line, which allow us to track his or her health status round-the-clock.
